Billing & Payment Policy
Choose either $200 monthly or $550 every 3 months.
Membership fees are automatically charged to the credit card, HSA, or FSA card on file.
Membership must remain active to continue receiving medication management, prescription refills, follow-up visits,
laboratory review, and portal messaging.
You may change your billing option or cancel your membership with 30 days' written notice before your next billing date.
Membership fees are non-refundable once medications have been prescribed, laboratory testing has been ordered, or
services have been provided.

Your membership is an investment in ongoing hormone optimization—not just prescriptions. Hormones require regular
monitoring, individualized dose adjustments, and continued support to help you achieve the best results safely and
effectively.

Membership Cancellation & Refund Policy
Memberships automatically renew according to the billing interval selected at enrollment unless canceled in accordance with the membership agreement.
Members may request cancellation by contacting Dynamic NP Functional Wellness at least 30 days before their next renewal date.
Payments for medications, supplements, laboratory testing, or other products/services that have already been ordered, dispensed, shipped, performed, or otherwise incurred on the patient's behalf are non-refundable to the extent permitted by applicable law.
Cancellation of a membership does not automatically cancel previously placed pharmacy, laboratory, or product orders.
